Looks like this was generated because the category was set to private, it worked when I turned off privacy.
 
Thanks, any idea why a thread with only embedded images mirrored all but a thread with both attached (2) and embedded (12) only mirrored the two attached images?

No, size should be ignored when mirroring existing attachments.
Make sure all attachments have a valid extension as specified in XFMG "Allowed image extensions" option
 
All images have .jpg extensions. Here are links to examples.

Do you have any other addons that affect attachments or media? Perhaps some additional restrictions do not allow mirroring them.

Thanks for the help. The problem was related to posts that had attachments that were not correct because of a problem a few years ago when I migrated from VB 3.8 to Xenforo.
