Hi, I read through readme as well as on xenforo and here, but I'm not seeing any documentation on configuration. I have configured what I believe would be a successful form, however I'm getting 404 errors and not quite sure why this is failing.

^^ I named the form 'whitelist application' and upon hitting the direct url it fails. This is not located here, I'm not certain where it gets created and how. Any help would be appreciated.
